What is Home Health Care?
Sutter Auburn Faith Visiting Nurse Association has a comprehensive range of home care professionals, products, and services that provide patients the ability to stay home or return home to recover from a medical condition, surgery, or exacerbation of a chronic illness.
The purpose of home health is to assess, teach and assist patients, and their caregivers to achieve their goal of living safely in a community setting.
What is Hospice Care?
Hospiceis a way of caring that brings comfort to persons living with a terminal illness. Hospice gives support to patients and their families. Care can be provided in the private home or in a care facility.
The hospice team provides effective pain management and symptom control along with psychosocial, emotional and spiritual support. The Hospice team respects the patient and family unit's right to make decisions about his or her own care.
Hospice neither hastens nor postpones death, but affirms life, emphasizing quality, not length.
The goal of Hospice is to enable terminally ill patients to live life as fully as possible with a spirit of hope and a sense of personal satisfaction and worth.
Admission Criteria
Home Health
- Patient requires the services of a licensed clinical professional (RN, PT, ST)
- Physician orders are required
- Care is provided on an intermittent basis
- Patient must be homebound
- Patient has a life limiting illness with a life expectancy of 6 months or less
- Curative medical intervention is no longer desired
- Symptom management is emphasized
- A caregiver must be available in prviate residential settings
